Anteflexion-Adduction Test

This test is indicated for:

  • Limited abduction.
  • Abnormal scapulo-humeral rhythm.
  • A diagnosis of ‘painful arc’ during the abduction movements.

Procedure

  • Ask the patient to bilaterally and simultaneously, carry out a maximal active anteflexion movement of 180° (if not possible, attempt passive movement) [Figure 55].

Figure 55

  • From this position, instruct the patient to return both arms slowly through the frontal plane (adduction movement) back to the starting position [Figure 56].

  • Pay attention to:
    • The course of the movements, particularly uniformity and smoothness.
    • The occurrence of pain and the possible presence of a ‘painful arc’ or ‘painful drop.’

This test is not possible with a frozen shoulder.
