Cause of the pain may be in a location other than that indicated by the child. For example, abdominal pain can be caused by middle-ear infection.<\/span><\/li>\n<li><span style=\"color: #003366;\">The <span style=\"text-decoration: underline;\">past history<\/span> always deserves attention, including the child\u2019s <span style=\"color: #33cccc;\"><strong>birth<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>development<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>growth<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>illnesses<\/strong><\/span>, and <span style=\"color: #33cccc;\"><strong>vaccinations<\/strong><\/span>.<\/span><\/li>\n<li><span style=\"color: #003366;\">Observe the child and their <span style=\"color: #33cccc;\"><strong>interaction<\/strong><\/span> with parents\/carers during the history-taking.<\/span><\/li>\n<\/ul>\n<hr \/>\n<h3><span style=\"color: #003366;\"><strong>Physical Examination<\/strong><\/span><\/h3>\n<p><span style=\"color: #003366;\">Form a <span style=\"color: #33cccc;\"><strong>general impression<\/strong><\/span> at the beginning of the examination. If the child is sick, how severe is the illness? If the child appears sick, always check <span style=\"color: #33cccc;\"><strong>vital functions<\/strong><\/span> first. Determine which examination has priority, sometimes <span style=\"color: #33cccc;\"><strong>rapid intervention<\/strong><\/span> is necessary, and a more detailed examination can be performed later.<\/span><\/p>\n<p><span style=\"color: #003366;\">In addition to the <span style=\"color: #33cccc;\"><strong>pulse<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>respiration<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>consciousness<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>temperature<\/strong><\/span>, and <span style=\"color: #33cccc;\"><strong>blood pressure<\/strong><\/span>; also measure the child\u2019s <span style=\"color: #33cccc;\"><strong>height<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>weight<\/strong><\/span>.<\/span><\/p>\n<p><span style=\"color: #003366;\">In general, examination of the child is usually broader than that for an adult with the same symptoms. For example, a child with abdominal pain may also undergo ear\/nose\/throat examination and pulmonary examination.<\/span><\/p>\n<p><span style=\"color: #003366;\">When examining a child, start at the <span style=\"color: #33cccc;\"><strong>top<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>work downward<\/strong><\/span>. <\/span><span style=\"color: #003366;\">Children are more likely to perceive the examination as <span style=\"color: #33cccc;\"><strong>threatening<\/strong><\/span>, particularly with the use of instruments. Give children <span style=\"color: #33cccc;\"><strong>time<\/strong><\/span> to adjust. For example, let them examine or even hold the stethoscope, let them try it first on the back of the hand, or demonstrate the examination on a toy.<\/span><\/p>\n<p><span style=\"color: #003366;\">Inspection of the <span style=\"color: #33cccc;\"><strong>groin<\/strong><\/span> (<em>inguinal or femoral hernia<\/em>) and <span style=\"color: #33cccc;\"><strong>scrotum<\/strong><\/span> (<em>undescended testes, testicular torsion<\/em>) are part of the abdominal examination. Inguinal hernias can also occur in girls.<\/span><\/p>\n<p><span style=\"color: #003366;\">Save unpleasant or painful examinations for last (<em>ear\/nose\/throat examination, rectal examination<\/em>). <\/span><span style=\"color: #003366;\">Take advantage of favourable moments during the examination. For example, check the throat while the child is crying and auscultate while the child is calm. <\/span><span style=\"color: #003366;\">Distract a crying child (<em>pacifier, bottle, game<\/em>) or let the child sit on the lap of the mother\/carer.
