{"id":777,"date":"2010-12-02T16:51:36","date_gmt":"2010-12-02T16:51:36","guid":{"rendered":"https:\/\/cloverock.info\/mockosce23\/new\/traumatic-inflammation\/"},"modified":"2024-06-13T14:54:47","modified_gmt":"2024-06-13T13:54:47","slug":"traumatic-inflammation","status":"publish","type":"post","link":"https:\/\/www.qpercom.com\/sim\/index.php\/minor-surgery\/theoretical-background\/traumatic-inflammation\/","title":{"rendered":"Traumatic Inflammation"},"content":{"rendered":"<hr \/>\n<p><span style=\"color: #003366;\"><span style=\"color: #33cccc;\"><strong>Overuse<\/strong><\/span> or <span style=\"color: #33cccc;\"><strong>trauma<\/strong><\/span> can lead to <span style=\"color: #33cccc;\"><strong>inflammatory reactions<\/strong><\/span> in the <span style=\"text-decoration: underline;\">periosteum<\/span>, <span style=\"text-decoration: underline;\">tendons<\/span>, <span style=\"text-decoration: underline;\">muscles<\/span>, and <span style=\"text-decoration: underline;\">bursae<\/span>.<\/span><\/p>\n<p><span style=\"color: #003366;\">Conservative treatment options for traumatic inflammation include <span style=\"color: #33cccc;\"><strong>rest<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>immobilisation<\/strong><\/span>. During the acute phase, a <span style=\"text-decoration: underline;\">pressure bandage<\/span> and <span style=\"text-decoration: underline;\">pharmacological therapy<\/span> (<em>antiphlogistics, analgesics, local injection of corticosteroids<\/em>) may be used. During later stages, <span style=\"text-decoration: underline;\">remedial therapy<\/span> can be considered.<\/span><\/p>\n<p><span style=\"color: #003366;\">The following types of traumatic inflammatory reactions are suitable for \u201c<em>surgical<\/em>\u201d treatment by a general practitioner.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><em><strong>Bursitis<\/strong><\/em><\/span><\/p>\n<p><span style=\"color: #003366;\">Inflammation of a <span style=\"color: #33cccc;\"><strong>bursa<\/strong><\/span>. Common types caused by trauma include <span style=\"text-decoration: underline;\">bursitis olecrani<\/span> and <span style=\"text-decoration: underline;\">bursitis praepatellaris<\/span>.<\/span><\/p>\n<p><span style=\"color: #003366;\"><strong><em>Therapy<\/em> [Decision Tree 2]:<\/strong> If you detect this problem in an early stage, a reasonable initial approach is watchful waiting. If the swelling has persisted for more than one week without improving, you may puncture it to rule out secondary infection (<em>pus<\/em>). If a diagnosis of traumatic inflammation without secondary infection is made (<em>this is the case when a clear, yellow liquid is aspirated<\/em>), then the bursa can be completely <span style=\"color: #33cccc;\"><strong>aspirated<\/strong><\/span> followed by <span style=\"color: #33cccc;\"><strong>hydrocortisone injection<\/strong><\/span>. Injection of hydrocortisone is contra-indicated when blood and\/or pus are aspirated. Infectious bursitis should be treated by <span style=\"color: #33cccc;\"><strong>incision<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>drainage<\/strong><\/span>.<\/span><\/p>\n<p><span style=\"color: #003366;\"><em>Puncturing, even when performed well, can also cause secondary infection.<\/em><\/span><\/p>\n<p><span style=\"color: #003366;\"><em><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-776\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/MinorSurgery_tree-2-400p.jpg\" alt=\"\" width=\"300\" height=\"321\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/MinorSurgery_tree-2-400p.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/MinorSurgery_tree-2-400p-280x300.jpg 280w\" sizes=\"auto, (max-width: 300px) 100vw, 300px\" \/><\/span><\/em><\/span><\/p>\n<p><span style=\"color: #003366;\"><strong><span class=\"jce_caption\" style=\"display: inline-block;\"><span style=\"width: 400px; display: block;\">Decision Tree 2<\/span><\/span><\/strong><em><br \/>\n<\/em><\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><em><strong>Epicondylitis Lateralis<\/strong><\/em><\/span><\/p>\n<p><span style=\"color: #003366;\">\u201c<em>Tennis Elbow<\/em>\u201d &#8211; micro-trauma occurring in the attachments of <span style=\"color: #33cccc;\"><strong>tendons<\/strong><\/span> of the extensor muscles due to <span style=\"color: #33cccc;\"><strong>stress<\/strong><\/span>.<\/span><\/p>\n<p><strong><span style=\"color: #003366;\"><em>Therapy: <\/em><\/span><\/strong><\/p>\n<ul>\n<li><span style=\"color: #003366;\">Rest.<\/span><\/li>\n<li><span style=\"color: #003366;\">Physiotherapy. The physiotherapist preferably uses a <span style=\"color: #33cccc;\"><strong>combined therapy<\/strong><\/span> approach with ultrasound and cross friction (<em>Cyriax method<\/em>). Both methods promote <span style=\"color: #33cccc;\"><strong>hyperaemia<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>ameliorate pain<\/strong><\/span> by <span style=\"text-decoration: underline;\">generating vibrations<\/span>.<\/span><\/li>\n<li><span style=\"color: #003366;\">Hydrocortisone injection.<\/span><\/li>\n<li><span style=\"color: #003366;\">Tenotomy by a surgeon.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #003366;\"><em><strong>Tendovaginitis Stenosans<\/strong><\/em><\/span><\/p>\n<p><span style=\"color: #003366;\">Inflammation of the <span style=\"color: #33cccc;\"><strong>tendon sheath<\/strong><\/span>, whereby the tendon thickens or the tendon sheath narrows, which <span style=\"text-decoration: underline;\">limits mobility<\/span>. When the tendons of the <span style=\"text-decoration: underline;\">m. abductor pollicis longus<\/span> and the <span style=\"text-decoration: underline;\">m. extensor pollicis brevis<\/span> are affected, it is referred to as <span style=\"color: #33cccc;\"><strong>De Quervain syndrome<\/strong><\/span>. If a tendon sheath of the flexors of the hand is affected, it is called \u201c<em>trigger finger<\/em><\/span><span style=\"color: #003366;\">.<\/span><span style=\"color: #003366;\">\u201d This name is derived from the snapping noise heard during passive finger extension. With this disorder, the flexor tendon thickens at the meta-carpophalangeal joint; the thickening is often palpable at this location.<\/span><\/p>\n<p><strong><span style=\"color: #003366;\"><em>Therapy: <\/em><\/span><\/strong><\/p>\n<ul>\n<li><span style=\"color: #003366;\">Hydrocortisone with lidocaine injection in the tendon sheath.<\/span><\/li>\n<li><span style=\"color: #003366;\">Recurrence cleavage of the tendon sheath by a surgeon.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Overuse or trauma can lead to inflammatory reactions in the periosteum, tendons, muscles, and bursae.
