{"id":76,"date":"2010-06-09T10:26:29","date_gmt":"2010-06-09T09:26:29","guid":{"rendered":"https:\/\/cloverock.info\/mockosce23\/new\/patients-suffering-from-brachialgia\/"},"modified":"2023-01-11T23:55:07","modified_gmt":"2023-01-11T23:55:07","slug":"patients-suffering-from-brachialgia","status":"publish","type":"post","link":"https:\/\/www.qpercom.com\/sim\/index.php\/the-neurological-examination\/patients-suffering-from-brachialgia\/patients-suffering-from-brachialgia\/","title":{"rendered":"Patients suffering from brachialgia"},"content":{"rendered":"<p><strong>Introduction<\/strong><br \/>\nBrachialgia \u2013 radiating pain in the arm \u2013 is a symptom that can be caused by cervical nerve root compression, but can also be caused by damage (usually compression) along the pathway of peripheral nerves in the arm. Nerve root compression is, in most cases, caused by a herniated cervical disc but, similar to lumbar nerve root compression, can also be caused by spondylarthrosis, spinal canal stenosis, and tumours etc. More peripheral nerve damage can be caused by constriction of the thoracic outlet (thoracic outlet syndrome) due to narrowing of the space between the scalene muscles, a cervical rib, and too little space between the clavicle and the first rib [Figure 124]. Compression in the elbow (median nerve under the pronator teres muscle) or the wrist (carpal tunnel syndrome) is possible.<br \/>\nConditions involving the shoulder or elbow in which nerves or plexus are not directly involved, can also cause shoulder and arm pain. With these conditions the nature of the pain is different, but since the nature of pain is often difficult for a patient to describe, joints and muscles will also have to be examined (refer to the publication entitled \u2018The Examination of the Upper Extremities\u2019 from this series).<\/p>\n<p><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-71\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/06\/NeuroExam_n-fig-124.jpg\" alt=\"\" width=\"400\" height=\"366\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/06\/NeuroExam_n-fig-124.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/06\/NeuroExam_n-fig-124-300x275.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><span style=\"width: 400px; display: block;\">Figure 124<\/span><\/span><\/p>\n<h2>Procedure<\/h2>\n<p><strong>Inspection<\/strong><\/p>\n<ul>\n<li>Ask the patient to undress the upper half of their body.&nbsp;<\/li>\n<li>Assess the head position.&nbsp;<\/li>\n<li>Assess whether there is any atrophy of the shoulder, arm and\/or hand muscles.<\/li>\n<\/ul>\n<p><strong><em>Examination of neck movement<\/em><\/strong><\/p>\n<ul>\n<li>Have the patient bend their neck to the left, right, forwards and backwards.<\/li>\n<li>Have the patient rotate their neck.&nbsp;<\/li>\n<li>Keep asking whether it hurts, where it hurts, whether the pain radiates and if so, to where.<\/li>\n<\/ul>\n<p><strong><em>Palpation of the neck<\/em><\/strong><\/p>\n<ul>\n<ul>\n<li>Palpate paravertebrally at the points where the C4-Th1 nerve roots exit.<\/li>\n<li>Ask whether this hurts, whether the pain radiates and if so, where to.<\/li>\n<\/ul>\n<\/ul>\n<p><em>&nbsp;<\/em><\/p>\n<p><em><strong>Examination of muscle strength<\/strong><\/em><br \/>\nAs described in the section &#8216;Motor function&#8217;, examine the strength of the following muscles: anterior serratus muscle, deltoid muscle, biceps muscle, triceps muscle of the arm, wrist extensor muscles, wrist flexor muscles, finger flexor muscles, finger extensor muscles, pollicis opponens muscle, interosseous muscles.<\/p>\n<p><em><strong>Examination of sensory function<\/strong><\/em><br \/>\nAs described in the section &#8216;Sensory function&#8217;, examine the spinothalamic and posterior column sensory functions in the C4-Th1 dermatomes.<\/p>\n<p><em><strong>Examination of the reflexes<\/strong><\/em><br \/>\nAs described in the section &#8216;The reflexes&#8217;, examine the biceps tendon reflex, the triceps tendon reflex and the radial reflex.<\/p>\n<p><em><strong>The nerve root irritation tests <\/strong><\/em><br \/>\nBy examining movement and palpating the cervical vertebral column, you should try to provoke the patient\u2019s symptom. The predictive value of the provocation tests used to confirm or rule out carpal tunnel syndrome has been tested and found to be very smaal. If there is loss of muscle strength, neurogenic pain is more likely, but pain can also result in an arm or shoulder being used less and therefore becoming less strong.
