It is generally understood to include a collection of <span style=\"color: #33cccc;\"><strong>limbs<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>joints<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>muscles<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>nerves<\/strong><\/span> and their <span style=\"color: #33cccc;\"><strong>functional interaction<\/strong><\/span>. The inseparable cohesion between the different organ systems makes it difficult to make a primary distinction between an <span style=\"text-decoration: underline;\">orthopaedic<\/span>, <span style=\"text-decoration: underline;\">neurological<\/span>, <span style=\"text-decoration: underline;\">vascular<\/span>, <span style=\"text-decoration: underline;\">psychogenic<\/span> or <span style=\"text-decoration: underline;\">internal medicine-based<\/span> causes of a complaint. Consequently, the diagnosis established is often no more than guesswork packaged in an impressive sounding name (<em>PHS, lumbago<\/em>), which covers a complex of poorly-understood complaints. Therefore, <span style=\"color: #33cccc;\"><strong>systematic<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>conscientious<\/strong><\/span> history-taking often provides the key to the mechanisms that ultimately lie behind the complaint.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\">Although this program is intended as a manual for practical skills, we certainly do not wish to ignore the history-taking. In the majority of cases, <span style=\"text-decoration: underline;\">pain<\/span> is the symptom which causes the patient to visit their GP. Ideally, during the process of clarifying the problem the patient should be given the opportunity to clearly <span style=\"color: #33cccc;\"><strong>state the nature of the pain<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>how this is experienced<\/strong><\/span>, the <span style=\"color: #33cccc;\"><strong>extent to which the symptom affects daily living<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>what the patient expects from the doctor<\/strong><\/span>.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\">The specific history-taking is the phase in which questions are posed about the modalities of the symptom (<em>aetiology, provocation, course of the problem over time, triggers improvement or deterioration, associated symptoms, radiation<\/em>). In addition, the musculoskeletal system is assessed in more detail. Associated symptoms such as <span style=\"color: #33cccc;\"><strong>paresthesia<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>loss of strength<\/strong><\/span>, require further neurological and vascular examinations. Symptoms such as <span style=\"color: #33cccc;\"><strong>swelling<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>redness<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>limited movement<\/strong><\/span> are indicative of an <span style=\"text-decoration: underline;\">inflammatory process<\/span> and necessitate additional <span style=\"text-decoration: underline;\">laboratory tests<\/span>.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\">Note that the concomitant occurrence of symptoms that may initially appear to be unrelated (<em>such as joint complaints and urethritis and\/or eye complaints<\/em>), might point towards the existence of a <span style=\"text-decoration: underline;\">specific syndrome<\/span> and can form a reason for further exploration by an <span style=\"text-decoration: underline;\">internal medicine specialist<\/span>.<\/span><\/p>\n<p><span style=\"color: #003366;\">There does not always have to be a direct relationship between the <span style=\"color: #33cccc;\"><strong>site<\/strong><\/span> where the pain is experienced and the <span style=\"color: #33cccc;\"><strong>location<\/strong><\/span> of the pain-inducing process (<em>referred pain<\/em>). Sometimes there are good reasons for enquiring about certain organ systems in further detail during the history-taking due to the possible presence of, for example, <span style=\"color: #33cccc;\"><strong>bone metastases<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>intermittent claudication<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>miction disorders<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>genital pathology<\/strong><\/span> etc.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\">The examination of the musculoskeletal system is divided into several subsidiary examinations. These will be discussed below in an order based on two premises.<\/span><\/p>\n<p><strong><span style=\"color: #003366;\">1.\u00a0 The examination should be performed systematically based on functional anatomical insights.<\/span><\/strong><\/p>\n<p><span style=\"color: #003366;\">The examination covers the following elements, listed in the order in which they should be carried out:<\/span><\/p>\n<ul>\n<li><span style=\"color: #003366;\">General inspection.<\/span><\/li>\n<li><span style=\"color: #003366;\">Basic functional examination:<\/span>\n<ul>\n<li><span style=\"color: #003366;\">Active movement (<em>function<\/em>) examination.<\/span><\/li>\n<li><span style=\"color: #003366;\">Passive movement (<em>function<\/em>) examination.<\/span><\/li>\n<li><span style=\"color: #003366;\">Muscle tests.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #003366;\">Palpation.<\/span><\/li>\n<li><span style=\"color: #003366;\">Specific tests.<\/span><\/li>\n<li><span style=\"color: #003366;\">Circumference measurement.<\/span><\/li>\n<li><span style=\"color: #003366;\">Neurological and vascular examinations.<\/span><\/li>\n<\/ul>\n<p><strong><span style=\"color: #003366;\">2.\u00a0 The inconvenience caused to the patient should be minimised by making full use of the patient\u2019s initial posture (<em>standing, sitting, or lying<\/em>). <\/span><\/strong><\/p>\n<p><span style=\"color: #003366;\">If the sequence of the examination is adhered to too strictly, it may disrupt the relaxed nature of the examination and place additional burden on the patient. Not all of the subsidiary examinations stated are always indicated.
