This can take on worrying forms in the pubertal growth spurt. A distinction has to be made between a <span style=\"text-decoration: underline;\">supple<\/span> exaggerated kyphosis and the <span style=\"text-decoration: underline;\">rigid<\/span> variety.\u00a0<\/span><\/p>\n<hr \/>\n<p><span class=\"jce_caption\" style=\"display: inline-block; color: #003366;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-716\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/Spine_sp-fig-67-v2.jpg\" width=\"400\" height=\"316\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/Spine_sp-fig-67-v2.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/Spine_sp-fig-67-v2-300x237.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><strong><span style=\"width: 400px; display: block;\">Figure 66<\/span><\/strong><\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><em><strong>A =<\/strong> Ideal posture<\/em><\/span><br \/>\n<span style=\"color: #003366;\"><em><strong>B =<\/strong> Kypholordosis position<\/em><\/span><br \/>\n<span style=\"color: #003366;\"><em><strong>C =<\/strong> Flat back<\/em><\/span><br \/>\n<span style=\"color: #003366;\"><em><strong>D =<\/strong> &#8216;Sway-back&#8217; position<\/em><\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\">The distinction can be made in a straightforward manner, using the following <strong>2 methods<\/strong>:<\/span><\/p>\n<ul>\n<li><strong><span style=\"color: #003366;\">In standing position:<\/span><\/strong><br \/>\n<span style=\"color: #003366;\">The patient often stands with <span style=\"color: #33cccc;\"><strong>increased lordosis<\/strong><\/span> and an <span style=\"color: #33cccc;\"><strong>overly tilted pelvis<\/strong><\/span>. By asking the patient to tilt the pelvis backwards, the lumbar lordosis will <span style=\"text-decoration: underline;\">reduce<\/span>. When the patient actively moves the shoulders backwards, stretching the <span style=\"color: #33cccc;\"><strong>cervical vertebral column<\/strong><\/span>, the thoracic kyphosis is <span style=\"text-decoration: underline;\">reduced<\/span> and an impression can be gained of the degree of flexibility of the kyphosis.<\/span><\/li>\n<li><strong><span style=\"color: #003366;\">In prone position:<\/span><\/strong><br \/>\n<span style=\"color: #003366;\">The <span style=\"color: #33cccc;\"><strong>spinous processes<\/strong><\/span> of the thoracic spine are <span style=\"text-decoration: underline;\">palpated<\/span> and an impression is gained of the degree of kyphosis. With a supple exaggerated kyphosis, it is usually already completely disappeared. By asking the patient to lift the shoulders, arms and the head, the kyphosis \u2013 if rigid \u2013 will not or will hardly reduce, whilst this is the case with a supple kyphosis.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #003366;\">The <span style=\"color: #33cccc;\"><strong>exaggerated kyphosis<\/strong><\/span> can be observed well from the <span style=\"text-decoration: underline;\">side<\/span> with the patient stooped forward, during which the shoulder blades slide forward and no longer obstruct the view onto the spine. In standing position, the <span style=\"color: #33cccc;\"><strong>lordosis depth<\/strong><\/span> (<em>distance between the plumb line past the top of the kyphosis and the lowest point of the lordosis<\/em>) and the <span style=\"color: #33cccc;\"><strong>sway-back distance<\/strong><\/span> (<em>distance between the plumb line past the top of the kyphosis and the upper side of the sacrum at the level of the start of the natal cleft<\/em>) can be measured. These measurements are conducted by an orthopaedist to estimate the severity of the kyphosis and, during follow-up of the patient, to assess the effect of therapeutic interventions.<\/span><\/p>\n<p><span style=\"color: #003366;\">If the kyphosis clearly shows rigid characteristics, <span style=\"color: #33cccc;\"><strong>osteochondrosis defects<\/strong><\/span> should be looked for on an x-ray image of an upright lateral total spine, which fits Scheuermann\u2019s disease or, more rarely, fusion of the front of the vertebrae (<em>congenital kyphosis<\/em>). Scheuermann\u2019s disease can often not be sufficiently corrected with posture therapy only and requires brace treatment in a growing child.<\/span><\/p>\n<p><span style=\"color: #003366;\">With <span style=\"color: #33cccc;\"><strong>supple posture kyphosis<\/strong><\/span>, <span style=\"color: #33cccc;\"><strong>exercise therapy<\/strong><\/span> or \u2018<em>watchful waiting<\/em>\u2019 will suffice.
