Never squirt directly in the direction of the tympanic membrane: as soon as sufficient cerumen has been removed, the water jet will hit the tympanic membrane with such force that there is a risk of perforation.<\/li>\n<li>Finally, carefully dry the outer part of the auditory meatus with a cotton bud [Figure 68].<\/li>\n<\/ul>\n<p><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-440\" style=\"margin: auto;\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-68.jpg\" alt=\"\" width=\"400\" height=\"299\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-68.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-68-300x224.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><span style=\"width: 400px; display: block;\">Figure 68<\/span><\/span><\/p>\n<p><em><strong>Using a cerumen loop or hook<\/strong><\/em><\/p>\n<p><iframe loading=\"lazy\" src=\"https:\/\/player.vimeo.com\/video\/122522910?dnt=1&amp;app_id=122963\" width=\"640\" height=\"480\" frameborder=\"0\" allow=\"autoplay; fullscreen; picture-in-picture\" allowfullscreen><\/iframe><\/p>\n<ul>\n<li>If you wish to remove cerumen in this manner, youwill have to look through the otoscope during the procedure to properly see what you are doing and to avoid damaging the auditory meatal wall or the tympanic membrane.<\/li>\n<li>Slide the retractable lens of the otoscope far enough to the side to allow the hook or loop to be inserted past it [Figure 69].<\/li>\n<\/ul>\n<p><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-441\" style=\"margin: auto;\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-69.jpg\" alt=\"\" width=\"400\" height=\"299\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-69.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-69-300x224.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><span style=\"width: 400px; display: block;\">Figure 69<\/span><\/span><\/p>\n<ul>\n<li>Attach a clean ear speculum to the head of the otoscope.<\/li>\n<li>Ask the patient to grasp the auricle with one hand and to pull it in a posterosuperior direction. Whilst looking through the partly retracted lens, insert the speculum into the auditory meatus [Figure 70].<\/li>\n<\/ul>\n<p><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-442\" style=\"margin: auto;\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-70.jpg\" alt=\"\" width=\"400\" height=\"299\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-70.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-70-300x224.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><span style=\"width: 400px; display: block;\">Figure 70<\/span><\/span><\/p>\n<ul>\n<li>When you have a good view of the cerumen plug, slide the loop or hook past the lens through the speculum into the auditory meatus [Figure 71]. Soft cerumen can be removed efficiently with a cerumen loop. If the cerumen is impacted, it is better to use a cerumen hook. The hook should be placed horizontally underneath the hard cerumen. Once you are sufficiently far under the cerumen (careful: you will not be able to see the tympanic membrane, so do not move too far), turn the hook upwards and retract it.<\/li>\n<\/ul>\n<p><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-443\" style=\"margin: auto;\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-71.jpg\" alt=\"\" width=\"400\" height=\"299\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-71.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/EarNoseMouth_enm-fig-71-300x224.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><span style=\"width: 400px; display: block;\">Figure 71<\/span><\/span><\/p>\n<p>For both the syringing and the cleaning technique using a cerumen loop or hook, it may be necessary to \u2018pre-soak\u2019 the cerumen plug. This can be done with standard baby oil.
