{"id":217,"date":"2010-12-02T11:30:33","date_gmt":"2010-12-02T11:30:33","guid":{"rendered":"https:\/\/cloverock.info\/mockosce23\/new\/deep-palpation\/"},"modified":"2023-01-11T23:33:21","modified_gmt":"2023-01-11T23:33:21","slug":"deep-palpation","status":"publish","type":"post","link":"https:\/\/www.qpercom.com\/sim\/index.php\/the-examination-of-the-abdomen\/the-abdominal-examination\/deep-palpation\/","title":{"rendered":"Deep palpation"},"content":{"rendered":"<h2>Procedure<\/h2>\n<p><iframe loading=\"lazy\" src=\"https:\/\/player.vimeo.com\/video\/122520418?dnt=1&amp;app_id=122963\" width=\"640\" height=\"480\" frameborder=\"0\" allow=\"autoplay; fullscreen; picture-in-picture\" allowfullscreen><\/iframe><\/p>\n<ul>\n<li>Place the hand on the abdomen as described in the section on superficial palpation<\/li>\n<li>Ensure that the abdominal muscles are as relaxed as possible<\/li>\n<li>Gradually apply more pressure and make small vertical movements with the flats of the fingers, pushing gently into the abdomen<\/li>\n<li>To palpate deeper, the non-dominant hand can be placed on top of the dominant hand with the finger tips at the level of the distal interphalangeal joints<br \/>\n[Figure 21]<\/li>\n<li>Distract the patient and palpate deeper on each expiration<\/li>\n<li>Palpate the entire abdomen<\/li>\n<li>Lastly, palpate the site of pain<\/li>\n<li>Check for any masses and palpation tenderness each time you press on the abdomen<\/li>\n<\/ul>\n<p><span class=\"jce_caption\" style=\"display: inline-block;\"><img loading=\"lazy\" decoding=\"async\" class=\" size-full wp-image-216\" style=\"margin: auto;\" src=\"https:\/\/cloverock.info\/mockosce23\/new\/wp-content\/uploads\/2010\/12\/Abdomen_ab-fig-21.jpg\" alt=\"\" width=\"400\" height=\"294\" srcset=\"https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/Abdomen_ab-fig-21.jpg 400w, https:\/\/www.qpercom.com\/sim\/wp-content\/uploads\/2010\/12\/Abdomen_ab-fig-21-300x221.jpg 300w\" sizes=\"auto, (max-width: 400px) 100vw, 400px\" \/><span style=\"width: 400px; display: block;\">Figure 21<\/span><\/span><\/p>\n<p><em><strong>Focus points<\/strong><\/em><\/p>\n<ul>\n<li>Determine where deep palpation is painful and record the perimeters of the tender areas found at palpation.<\/li>\n<li>If a mass is palpable, determine whether there is a possibility of a \u2018false\u2019 tumour, such as a gravid uterus, a full urinary bladder, or intestines filled with compacted faeces.<\/li>\n<li>It is sometimes difficult to determine whether the mass felt is located in the abdominal wall or abdominal cavity. How to differentiate between these two cases is described in the previous paragraph under \u2018superficial palpation\u2019.<\/li>\n<li>Assess a pathological mass based on the following aspects:\n<ul>\n<li>location<br \/>\n<em>Determine the precise location of the mass<\/em><\/li>\n<li>size<br \/>\n<em>Estimate the size in centimetres<\/em><\/li>\n<li>shape<br \/>\n<em>Describe the shape, using terms such as elongated, lobular, or round; describe the borders as accurately as possible<\/em><\/li>\n<li>surface<br \/>\n<em>Describe the surface as smooth or irregular and bumpy. Be aware that this can be difficult due to the interfering effects of the layer found between the palpating hand and the mass.<\/em><\/li>\n<li>consistency<br \/>\n<em>Describe the consistency as soft, rubbery, firm, very firm, or solid (only bone is solid)<\/em><\/li>\n<li>pulsations<br \/>\n<em>Differentiating between a pulsating swelling and pulsations transmitted through a mass to the abdominal wall can be very difficult in some cases.
