{"id":1752,"date":"2010-12-13T11:16:59","date_gmt":"2010-12-13T11:16:59","guid":{"rendered":"https:\/\/cloverock.info\/mockosce23\/new\/supervision-of-the-dilivery\/"},"modified":"2023-09-20T10:04:20","modified_gmt":"2023-09-20T09:04:20","slug":"supervision-of-the-dilivery","status":"publish","type":"post","link":"https:\/\/www.qpercom.com\/sim\/index.php\/obstetrics\/management-of-childbirth\/supervision-of-the-dilivery\/","title":{"rendered":"Supervision Of The Delivery"},"content":{"rendered":"<hr \/>\n<p><span style=\"color: #003366;\"><em><strong>Materials to be laid out in preparation: <\/strong><\/em><\/span><\/p>\n<ul>\n<li><span style=\"color: #003366;\">Monaural stethoscope or foetal doppler.<\/span><\/li>\n<li><span style=\"color: #003366;\">Sterile gloves.<\/span><\/li>\n<li><span style=\"color: #003366;\">Sterile cloths or washed and ironed cotton nappies.<\/span><\/li>\n<li><span style=\"color: #003366;\">Sterile gauzes 5 \u00d7 5 cm and 20 \u00d7 10 cm.<\/span><\/li>\n<li><span style=\"color: #003366;\">Bowl with cotton wall balls soaked in water or disinfectant.<\/span><\/li>\n<li><span style=\"color: #003366;\">Bowl or waste bucket.<\/span><\/li>\n<li><span style=\"color: #003366;\">2 Kocher clamps.<\/span><\/li>\n<li><span style=\"color: #003366;\">Episiotomy scissors or scalpel.<\/span><\/li>\n<li><span style=\"color: #003366;\">Placenta pan or bedpan.<\/span><\/li>\n<li><span style=\"color: #003366;\">Umbilical cord scissors.<\/span><\/li>\n<li><span style=\"color: #003366;\">Mucus suction device.<\/span><\/li>\n<li><span style=\"color: #003366;\">Umbilical cord clamp.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #003366;\"><em>The following materials do not need to be laid out but must be within easy reach:<\/em><\/span><\/p>\n<ul>\n<li><span style=\"color: #003366;\">Amniotomy instrument.<\/span><\/li>\n<li><span style=\"color: #003366;\">Sterile bladder catheter.<\/span><\/li>\n<li><span style=\"color: #003366;\">Injection syringes 1 ml, 2 ml, 10 ml, with needles.<\/span><\/li>\n<li><span style=\"color: #003366;\">Suture material.<\/span><\/li>\n<li><span style=\"color: #003366;\">Local anaesthetic.<\/span><\/li>\n<li><span style=\"color: #003366;\">Bag valve mask for oxygen administration.<\/span><\/li>\n<\/ul>\n<hr \/>\n<h3><span style=\"text-decoration: underline;\"><span style=\"color: #003366; text-decoration: underline;\">Procedure<\/span><\/span><\/h3>\n<ul>\n<li><span style=\"color: #003366;\">Monitor the foetal heart sounds after each contraction and note the frequency and rhythm.<\/span><\/li>\n<li><span style=\"color: #003366;\">Check the woman\u2019s blood pressure.<\/span><\/li>\n<li><span style=\"color: #003366;\">Tell the woman that she can push at will. Give the traditional pushing instructions if required:<\/span>\n<ul>\n<li><span style=\"color: #003366;\">During a contraction, push (<em>bear down<\/em>) as long and as hard as possible (<em>two to three times<\/em>).<\/span><\/li>\n<li><span style=\"color: #003366;\">Relax and rest in between contractions.<\/span><\/li>\n<li><span style=\"color: #003366;\">When a woman giving birth in the supine position feels a contraction coming, she should grab hold of her legs behind the knees and pull her legs back and slightly apart.<\/span><\/li>\n<li><span style=\"color: #003366;\">At the peak of the contraction, she should breathe as deeply as possible.<\/span><\/li>\n<li><span style=\"color: #003366;\">She should close her mouth and hold her breath.<\/span><\/li>\n<li><span style=\"color: #003366;\">She should put her chin down and keep her shoulders flat, or move to the preferred position, for example, half sitting.<\/span><\/li>\n<li><span style=\"color: #003366;\">Push\/bear down, as if to have a bowel movement, and prevent pressure build up in the head.<\/span><\/li>\n<li><span style=\"color: #003366;\">Push calmly and as long as possible, without screaming, until she needs to breathe in again.<\/span><\/li>\n<\/ul>\n<\/li>\n<li><span style=\"color: #003366;\">Palpate the uterus to assess the quality of the contractions.<\/span><\/li>\n<li><span style=\"color: #003366;\">Wait until the scalp appears.<\/span><\/li>\n<li><span style=\"color: #003366;\">Put on the sterile gloves.<\/span><\/li>\n<li><span style=\"color: #003366;\">Clean the vulva.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p><iframe loading=\"lazy\" src=\"https:\/\/player.vimeo.com\/video\/124023555?dnt=1&amp;app_id=122963\" width=\"640\" height=\"480\" frameborder=\"0\" allow=\"autoplay; fullscreen; picture-in-picture\"><\/iframe><\/p>\n<hr \/>\n<ul>\n<li><span style=\"color: #003366;\">Tell the woman that in several minutes time when you give the signal, she will have to alternate between pushing and panting.<\/span><\/li>\n<li><span style=\"color: #003366;\">Support the perineum with one hand with gauze (20 \u00d7 10 cm) so that you can continue to inspect it or keep your hand close to the perineum.<\/span><\/li>\n<li><span style=\"color: #003366;\">Once the scalp crowns, instruct the woman to stop pushing and to pant for the rest of the contraction.<\/span><\/li>\n<li><span style=\"color: #003366;\">Place your other hand on the visible part of the head or keep your hand close to this <em><strong>[Figure\u00a025]<\/strong><\/em>.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p><iframe loading=\"lazy\" src=\"https:\/\/player.vimeo.com\/video\/124023563?dnt=1&amp;app_id=122963\" width=\"640\" height=\"480\" frameborder=\"0\" allow=\"autoplay;
