Specula<\/strong>\u00a0<\/span><\/span><\/h2>\n

A vaginal speculum<\/strong><\/span> (duckbill speculum<\/em>) is used for spreading<\/span> the vaginal walls, which allows for the internal inspection<\/strong><\/span> or diagnostic procedure<\/strong><\/span> [Figure 3]<\/strong><\/em>. There are many different types of specula and all have their own advantages<\/em> and disadvantages<\/em>. The choice of speculum depends on the situation<\/em>, preference<\/em> and experience<\/em>. The most common ones are discussed below.<\/span><\/p>\n

Seyffert<\/strong>\u00a0<\/span><\/h3>\n

This speculum has a pistol grip<\/strong><\/span>, which is easy to handle<\/span>, however, the heaviness<\/em> of the grip, makes it prone to tilting<\/strong><\/span> [Figure 4]<\/strong><\/em>. It must always be held properly to prevent this. If both hands are required to hold it, it is possible to ask the patient to hold the top of the speculum. The lower blade<\/em> of the Seyffert speculum is longer<\/span> than the upper blade<\/em>. This makes it easier to gain a view of the vaginal portion of the cervix<\/span>, particularly when it is deep-lying<\/strong><\/span>. However, this speculum can only be used with the grip facing downwards<\/strong><\/span> (dorsally<\/em>). It can only be fixed in a few positions<\/em>, making it difficult to find the right balance between spreading too wide<\/span> and having an insufficient view<\/span>.<\/span><\/p>\n

Cusco<\/strong>\u00a0<\/span><\/h3>\n

This speculum is more lightly-equipped<\/strong><\/span>, looks \u2018friendlier<\/em>\u2019 and is easier<\/span> to handle [Figure 5]<\/strong><\/em>. The front<\/em> and rear blade<\/em> are of equal length<\/span>, so the speculum can be inserted both with the grip facing forward<\/em> and backward<\/em>. If the vaginal portion of the cervix is deep-lying, it may be hard to gain a view without a longer<\/span> lower blade. Fixing the speculum can be carried out in a stepless manner using a finger wheel.<\/span><\/p>\n

Semm<\/strong>\u00a0<\/span><\/h3>\n

This is another lightly-equipped<\/strong><\/span> speculum that looks \u2018friendlier<\/em>\u2019 and is easier<\/span> to handle [Figure 6]<\/strong><\/em>. This one has a lower<\/span> risk of tilting because of the lighter<\/span> handle. The front<\/em> and rear blade<\/em> are of equal length<\/span>. A deeper-lying vaginal portion of the cervix may be harder to see because of this, but this speculum can be inserted with the handle either facing forward<\/em> or backward<\/em>. Fixing the speculum can only be done in a number<\/em> of fixed positions, as with the Seyffert<\/em> speculum.<\/span><\/p>\n

PVC Speculum<\/strong><\/span><\/h3>\n

This speculum is disposable<\/strong><\/span> and sterile-packaged<\/strong><\/span>. It is portable<\/strong><\/span> and light<\/strong><\/span>, but also less sturdy<\/span> [Figure 7]<\/strong><\/em>. Additionally, these speculums can sometimes have sharp edges<\/span>. However, they never feel cold<\/span> and appear less bulky<\/span>. The blades may be layered with lubricant<\/strong><\/span>, which is activated once the blades are dampened<\/em> with water. If the blades of the speculum are transparent<\/span>, the inspection of the vaginal walls is easier<\/em>.<\/span><\/p>\n

An alternative to this speculum is the Welch Allyn speculum<\/strong><\/span>, to which a lamp can be fitted inside. Unfortunately, this makes the speculum heavier<\/span>.<\/em><\/span><\/p>\n

\n

The Cusco speculum<\/strong><\/span> is available with many different<\/span> types of blades, whereas the Seyffert<\/strong><\/span> and the Semm<\/span> are available only in a \u2018normal<\/em>\u2019 and a narrow variant [Figure 8]<\/strong><\/em>.<\/span><\/p>\n

After use, the specula and other metal instruments must be \u2018domestically<\/em>\u2019 cleaned <\/span>(washed thoroughly<\/em>). Prior to using the instruments again, they will have to be sterilised<\/strong><\/span>.<\/span><\/p>\n
