Experience and intuition form the cement that holds these building blocks together.<\/span><\/p>\n<hr \/>\n<ul>\n<li><span style=\"color: #003366;\">The most essential questions to be asked when taking the history, concern the <span style=\"color: #33cccc;\"><strong>immediate cause<\/strong><\/span> (<em>what exactly happened and under which circumstances<\/em>), <span style=\"color: #33cccc;\"><strong>timing<\/strong><\/span> (<em>when it happened<\/em>), <span style=\"color: #33cccc;\"><strong>any complicating factors<\/strong><\/span> (<em>diabetes, haemophilia, medication, imminent shock<\/em>) and <span style=\"color: #33cccc;\"><strong>tetanus prevention<\/strong><\/span>.<\/span><\/li>\n<li><span style=\"color: #003366;\">It may be necessary to <span style=\"text-decoration: underline;\">clean the wound<\/span> in order to inspect it. If the wound appears to be one which will require primary wound closure using a needle and thread, local anaesthetic can be applied immediately to ensure that cleaning and inspection are as painless as possible. For further details refer to the <span style=\"color: #33cccc;\"><strong>Minor Surgery<\/strong><\/span> edition in this series.<\/span><\/li>\n<li><span style=\"color: #003366;\">In some wounds, the tissue damage may be far <span style=\"text-decoration: underline;\">more serious<\/span> than suggested by the initial appearance. For example, in the case of <em>ruptures<\/em>, <em>sprains<\/em>, <em>stab<\/em>, <em>shot<\/em> and <em>burn wounds<\/em>, <em>fractures<\/em>, <em>irradiation<\/em> and <em>electrical injuries<\/em>. If there are no clear contraindications, and if possible nerve, muscle, tendon or ligament damage is suspected, a <span style=\"color: #33cccc;\"><strong>functional assessment<\/strong><\/span> should be carried out.
