The cause of this is usually a preferred intrauterine posture and the foot can therefore be passively rectified.<\/span><\/p>\n<p><span style=\"color: #003366;\">In terms of a <span style=\"color: #33cccc;\"><strong>pathological clubfoot<\/strong><\/span>, three components can be distinguished that can each occur separately or together:<\/span><\/p>\n<ul>\n<li><span style=\"color: #003366;\">A <span style=\"color: #33cccc;\"><strong>stretched position<\/strong><\/span> of the <span style=\"text-decoration: underline;\">ankle joint<\/span>.<\/span><\/li>\n<li><span style=\"color: #003366;\">A <span style=\"color: #33cccc;\"><strong>varus position<\/strong> <\/span>of the <span style=\"text-decoration: underline;\">rear foot<\/span>.<\/span><\/li>\n<li><span style=\"color: #003366;\">An <span style=\"color: #33cccc;\"><strong>adduction position<\/strong><\/span> of the <span style=\"text-decoration: underline;\">forefoot<\/span>.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p><span style=\"color: #003366;\">In 80% of the cases, the aetiology is unknown, yet there appears to be a <span style=\"color: #33cccc;\"><strong>familial tendency<\/strong><\/span>. In 20% of the cases, this abnormality is associated with other abnormalities, usually <span style=\"color: #33cccc;\"><strong>neurological<\/strong><\/span>. Children with clubfeet should quickly be referred to the orthopaedic surgeon.<\/span><br \/>\n<span style=\"color: #003366;\">After immobilisation in a plaster for a period of 3 to 4 months, the need for surgical correction will be examined.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><strong>Metatarsus Adductus<\/strong><\/span><\/p>\n<p><span style=\"color: #003366;\">The forefoot is <span style=\"color: #33cccc;\"><strong>adducted<\/strong><\/span> compared to the rest of the foot. The forefoot can passively be brought into the neutral position.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><strong>Metatarsus Varus<\/strong>\u00a0<\/span><\/p>\n<p><span style=\"color: #003366;\">The forefoot is in <span style=\"color: #33cccc;\"><strong>adduction<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>supination<\/strong><\/span>. There is also a <span style=\"color: #33cccc;\"><strong>high longitudinal arch<\/strong><\/span> and a <span style=\"color: #33cccc;\"><strong>deep skin fold<\/strong><\/span> on the transition between the forefoot and rear foot. Every child with metatarsus varus should be referred immediately, because it is not possible to completely correct the foot passively.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><strong>Pes Varus<\/strong><\/span><\/p>\n<p><span style=\"color: #003366;\">The forefoot is in <span style=\"color: #33cccc;\"><strong>adduction<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>supination<\/strong><\/span>, the rear foot is in the varus position. <span style=\"color: #33cccc;\"><strong>Dorsal flexion<\/strong><\/span> is fully possible. Without treatment, the child will walk on the lateral edge of the foot to an extreme extent.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><strong>Metatarsus Primus Varus<\/strong><\/span><\/p>\n<p><span style=\"color: #003366;\">The first metatarsal bone is <span style=\"color: #33cccc;\"><strong>adducted<\/strong><\/span> with respect to the rest of the foot and there is considerable space between <span style=\"color: #33cccc;\"><strong>digits I<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>II<\/strong><\/span>. This condition does not require treatment, although some suggest it has a causal relationship with the development of a <span style=\"color: #33cccc;\"><strong>hallux valgus<\/strong><\/span> at a more advanced age.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><strong>Talipes Calcaneus And Congenital Vertical Talus<\/strong><\/span><\/p>\n<p><span style=\"color: #003366;\">In the case of <span style=\"color: #33cccc;\"><strong>talipes calcaneus<\/strong><\/span>, the rear foot is in extreme <span style=\"color: #33cccc;\"><strong>dorsal flexion<\/strong><\/span> and slight <span style=\"color: #33cccc;\"><strong>valgus inclination<\/strong><\/span>. The sole of the foot is flat and the medial side of the head of the talus does not protrude. Talipes calcaneus that cannot be corrected must be immobilised in plaster.<\/span><\/p>\n<p><span style=\"color: #003366;\">Regarding <span style=\"color: #33cccc;\"><strong>congenital vertical talus<\/strong><\/span>, there is severe <span style=\"color: #33cccc;\"><strong>malformation<\/strong><\/span> of the root of the foot. The rear foot at the height of the ankle joint is in fixed <span style=\"color: #33cccc;\"><strong>equinus<\/strong><\/span>. The mid and forefoot are in <span style=\"color: #33cccc;\"><strong>dorsal flexion<\/strong><\/span> and <span style=\"color: #33cccc;\"><strong>abduction<\/strong><\/span> (<em>due to luxation in Chopart\u2019s joint<\/em>) with respect to the rear foot. The sole of the foot is <span style=\"color: #33cccc;\"><strong>convex<\/strong><\/span> instead of <span style=\"color: #33cccc;\"><strong>concave<\/strong><\/span> and the medial part of the head of the talus is clearly palpable. As this is a rigid defect, accompanied by many neuromuscular abnormalities, <span style=\"color: #33cccc;\"><strong>surgical intervention<\/strong><\/span> is indicated.<\/span><\/p>\n<hr \/>\n<p><span style=\"color: #003366;\"><strong>Hollow Foot<\/strong><\/span><\/p>\n<p><span style=\"color: #003366;\"><span style=\"color: #33cccc;\"><strong>Hollow foot<\/strong><\/span> is a foot defect in which the forefoot assumes a fixed <span style=\"color: #33cccc;\"><strong>equinus<\/strong><\/span> position with respect to the rear foot. In 75% of cases, this defect is associated with <span style=\"color: #33cccc;\"><strong>neuromuscular disorders<\/strong><\/span>. In the other 25% of cases, it concerns an <span style=\"color: #33cccc;\"><strong>idiopathic disorder<\/strong><\/span> with a familial tendency. The defect mostly does not develop until between the 5th and 10th year of life. It is believed that the cause lies in an imbalance between the <span style=\"color: #33cccc;\"><strong>intrinsic<\/strong><\/span> and the <span style=\"color: #33cccc;\"><strong>extrinsic musculature<\/strong><\/span> of the foot.<\/span><\/p>\n<p><span style=\"color: #003366;\">There are two types:<\/span><\/p>\n<ul>\n<li><span style=\"color: #003366;\"><em><span style=\"color: #33cccc;\"><strong>The simple cavus foot<\/strong><\/span>.<\/em> This is characterised by <span style=\"color: #33cccc;\"><strong>rigid equinus<\/strong><\/span> of both the medial and lateral foot shafts with respect to the rear foot, which results in a <span style=\"color: #33cccc;\"><strong>high instep<\/strong><\/span> with an excessively <span style=\"color: #33cccc;\"><strong>concave longitudinal arch<\/strong><\/span>. Pain may be indicated at the height of the medial arch. Hammer toes and claw toes are often secondary symptoms.<\/span><\/li>\n<li><span style=\"color: #003366;\"><em><span style=\"color: #33cccc;\"><strong>Cavovarus foot<\/strong><\/span>.<\/em> Only the medial foot shafts are in <span style=\"color: #33cccc;\"><strong>equinus<\/strong><\/span>, and a <span style=\"color: #33cccc;\"><strong>non-rigid varus position<\/strong><\/span> of the rear foot can develop as a secondary symptom. <span style=\"color: #33cccc;\"><strong>Soft tissue correction<\/strong><\/span> is recommended by means of releasing the plantar fascia if the condition causes significant symptoms.<\/span><\/li>\n<\/ul>\n<hr \/>\n<p>&nbsp;<\/p>\n","protected":false},"excerpt":{"rendered":"<p>Clubfoot A neonate can have a varus position of the foot without this being pathological.
